Finding Our Office

Our office is located on the second floor of Medical Plaza 2 on the Providence Willamette Falls Hospital campus, in Suite 205. To find our office, enter Medical Plaza 2 and take the stairs or elevator to the second floor.

Note that in the event of inclement weather, our office is only accessible by taking 7th St (Molalla Ave.) to Division St. Both Anchor Way and 15th St. will be closed.

By car: From I-205 north, take exit 9 for Oregon City. Turn left at the first light onto Highway 99E, then:

  • Left on 14th St.
  • Left on Washington St.
  • Right on 15th St., and follow it up the hill.
  • Left on Division St. and take an immediate right in the first driveway between the hospital and Medical Plaza 1.
  • Continue straight past Medical Plaza 2 and turn left into the parking lot.

From I-205 south, take exit 10 for Highway 213, Molalla Park Place. Turn right at the first light on Prairie Schooner Way (towards Home Depot), then:

  • Left on Washington St.
  • Left onto 15th St., and follow it up the hill
  • Left on Division St. and take an immediate right in the first driveway between the hospital and Medical Plaza 1.
  • Continue straight past Medical Plaza 2 and turn left into the parking lot.

There is a two-level, free parking garage located behind the Medical Plaza 2 building. You can access the upper level from the hospital driveway or the lower level from Davis St. You can also park right off Division St. in front of Medical Plaza 1 and use the skybridge to get to Medical Plaza 2.

This location is wheelchair accessible with accessible parking available near the building entrance and in the parking garage. Both the garage and the medical office building have elevator access to each floor. If you have specific accessibility needs, please call our office before your visit or let our staff know when you arrive so we can help accommodate you.

Free interpreter services are available, including ASL. To request an interpreter, call our office before your visit and tell us your preferred language. You may also ask our staff for language assistance when you arrive. For more information, see our Language Access & Non-discrimination notice.

WHA accepts Medicaid, Medicare and most commercial insurance plans. We participate in many plans commonly used in the Portland area. Review accepted insurance plans.

If you’re pregnant and want to begin care at WHA–or need to schedule an appointment during your pregnancy, call us! Find a provider and location here.

Please have your insurance information handy before you begin scheduling.

This will allow our teams to check whether your plan may require a referral for the care you need or to prepare benefits information to share with you at your visit. If you do not enter insurance information when scheduling, you may be asked to pay a $200-$500 deposit before receiving service.
