Patient Information

Before you come see us on your first visit:

  • Please verify that the Women's Healthcare Associates clinician you wish to see is contracted with your insurance plan.
  • If needed, obtain a referral from your primary care physician.
  • Make payment arrangements if you do not have insurance coverage.
  • Write down any questions or concerns you wish to discuss with us.

On the day of your appointment, please bring:

  • Your insurance card
  • Your completed Patient Information form
    English | Spanish (pdf*)
  • Your completed Medical History form (pdf*)
  • Your list of questions or concerns
  • A deposit for services rendered, unless your insurance plan is an HMO or PPO, in which case we request your co-payment only. We accept checks and credit cards (Visa and Mastercard).

About your appointment:

Except for urgent medical situations, we see patients by appointment only. We will work hard to honor your appointment time and intend to keep you informed of any delay. Should your clinician be called away for a delivery or medical emergency, you will be offered the option of rescheduling your appointment, waiting to see your clinician, or seeing another clinician who is available in the office. Please be assured that all of our clinicians offer the same excellent level of service.

If you are not able to keep your appointment, please notify your Women's Healthcare Associates clinician. We appreciate 24 hours notice.

Insurance:

We participate in most insurance plans that serve the Portland area. To verify participation with your insurance carrier, please contact your health plan or the patient account representative at your clinician's office.

Billing Information:

For our newly pregnant patients-We will meet with you at our office to review and discuss your insurance coverage. We will also answer questions you may have about your financial obligations during your pregnancy.

For patients without insurance coverage-If you do not have insurance coverage or your insurance card with you, you will be expected to make financial arrangements, including a deposit on the day of your appointment.

We also offer cash discounts if you pay your entire bill during your first appointment.

Paying your bill:

You will receive a statement from us showing your balance after insurance payments. Your bill is due and payable within 30 days after the billing date. For your convenience, we accept personal checks, Visa and Mastercard. If your insurance company has not paid within 45 days, please contact your insurance company.

Medical Record Release Form:

This helps us gather your records from your previous healthcare provider so that you don't have to repeat a lot of your historical information.

HIPAA (Health Insurance Portability and Accountability Act):
