New Patient Checklist

Remember when mom made sure you had everything you needed? Think of this as mom’s checklist—only (a little) shorter.

Before your first appointment:

  • Check your email for an invitation to set up your MyHealth account from [email protected]. Follow the instructions to activate your account.
  • Once your MyHealth account is activated, check to see if you have any forms to complete before your visit.
  • Verify that the WHA provider you wish to see is contracted with your insurance plan. If needed, obtain a referral from your primary care physician.
  • If you’re coming to see us for a wellness visit and any related screening (such as a screening mammogram), verify that you meet any requirements your insurance company has set; for example, some insurance plans are really specific about how often they allow you to have wellness visits. If your plan says once every 365 days and your appointment is on day 364, you could be responsible for the cost of the visit.
  • Make payment arrangements if you do not have insurance coverage.
  • Write down any questions or concerns you wish to discuss with us.

On the day of your appointment, please bring:

  • Your insurance card and photo identification.
  • A list of questions or concerns you may have.
  • A list of all medications you are currently taking.
  • A deposit for services, unless your insurance plan is an HMO or PPO, in which case we request your co-payment only. We accept checks and credit cards (Visa, Mastercard, American Express and Discover).

Please have your insurance information handy before you begin scheduling.

This will allow our teams to check whether your plan may require a referral for the care you need or to prepare benefits information to share with you at your visit. If you do not enter insurance information when scheduling, you may be asked to pay a $200-$500 deposit before receiving service.
